What it's for
This T&M ticket template records one piece of time-and-material work: the labor, the equipment and the materials it took, with the amounts and the total worked out on the sheet. A foreman fills it in as the work is done, the customer's representative signs it, and the office carries the total to the bill.
Use one ticket for each piece of work, and name the change order or RFI it belongs to under CHANGE ORDER OR RFI REFERENCE. The ticket records what was done; it doesn't write the change up. What's inside
Two sheets, both protected with no password. Pale yellow cells are yours to fill in; all other cells are locked.
Ticket is the form: your letterhead, printed "T&M TICKET," with TICKET NO. and DATE, then:
- JOB: PROJECT, CUSTOMER OR GENERAL CONTRACTOR, LOCATION ON SITE, AUTHORIZED BY and CHANGE ORDER OR RFI REFERENCE.
- DESCRIPTION OF WORK: three lines.
- LABOR: 6 lines of WORKER OR CLASS, HOURS, RATE and AMOUNT.
- EQUIPMENT: 3 lines of EQUIPMENT OR TOOL, HOURS, RATE and AMOUNT.
- MATERIALS: 8 lines of ITEM, UNIT, QTY, UNIT COST and AMOUNT.
- SUMMARY: Labor, Labor markup, Equipment, Equipment markup, Materials, Materials markup, Sales tax on materials and a highlighted Total.
- SIGN-OFF: WORK PERFORMED BY and CUSTOMER'S REPRESENTATIVE, each with a signature box, a NAME and a DATE.
Every AMOUNT calculates. The ticket prints on two Letter pages, with Materials starting page 2 under a repeat of the letterhead.

How to fill it in
- Type your details over the yellow letterhead, then enter TICKET NO. and DATE. To remove the YOUR LOGO text, choose Review > Unprotect Sheet (there is no password), then protect the sheet again.
- Fill in the five JOB fields and describe the work under DESCRIPTION OF WORK.
- Clear or replace the example lines first. Each subtotal adds every line in its table, so anything left in counts.
- List the labor by WORKER OR CLASS, and the equipment, each with HOURS and RATE. AMOUNT is hours times rate, rounded to the cent.
- List each material's ITEM, UNIT, QTY and UNIT COST. AMOUNT is quantity times unit cost. The number columns refuse text.
- In the summary, type a rate beside Labor markup, Equipment markup and Materials markup, and beside Sales tax on materials if it applies. Enter each as a percent, such as 10%; the cell takes 0% to 100%. The rates come from your agreement, and the only markup or tax rate in the file is the labeled 10% example. Blank means 0.
- Read the Total, then have the crew and the customer's representative sign, with NAME and DATE.
A labor or equipment line counts once its name, hours or rate is filled in, and a materials line once its item, quantity or unit cost is, so a worker with hours and no rate shows $0.00 until the rate goes in. Each markup is figured on its own subtotal. Sales tax is the Materials total times its rate, before any markup, and the file doesn't decide which items your state taxes or at what rate. The worked example
The file opens on two labor lines, two materials lines and a 10% materials markup, each labeled Example. The About tab calls them illustrative figures, not typical rates, prices or markups. Labor is 3 hours at $78.00 ($234.00) and 3 hours at $62.00 ($186.00). Materials are 12 ea at $7.25 ($87.00) and 1 box at $48.00 ($48.00).

Frequently asked questions
Does the file do the math?
Yes. Every amount, subtotal, markup, the sales tax and the total is a live formula, rounded to the cent.
What markup should I enter?
The one your agreement sets. The file starts every markup and tax rate blank except the 10% shown as an example on the materials line, and it suggests no figure.
Does it work in Google Sheets or Numbers?
Both open .xlsx files. This one has no macros, and its formulas use only IF, AND, ROUND and SUM.
Can I add lines?
Yes. Unprotect the sheet (there's no password), insert rows between two lines of a table so its subtotal takes them in, copy the AMOUNT formula into each new row, then protect the sheet again. Check the print preview afterward.
Can I put my logo on it?
Yes. Insert it where the sheet says YOUR LOGO. That row is 36 pt (half an inch) tall, so a logo sized to it fits without dragging the row taller. To remove the YOUR LOGO text, choose Review > Unprotect Sheet (there is no password), then protect the sheet again.
Is it legal advice?
No. The ticket records hours, quantities and costs and holds the signatures. It states no terms, and it doesn't say what your agreement allows. If you want your terms reviewed, ask your attorney.
