What it's for

This notice to proceed template is a one-page form that tells a contractor, in writing, that work on a job may start on a stated date. Fill it in when the start date is set, send it, and file the signed copy with the job.

The file is a notice with blanks that restates no contract terms, and this page gives no legal advice.

The form names the agreement it goes with and carries no description of the work. Describe the work on a scope of work, and use the construction meeting minutes template for the preconstruction meeting the form has a line for: filled in ahead of time, the minutes form works as its agenda.

What's inside

One Letter-size page, with our logo, a credit link and the page number in the footer. The pale yellow boxes are yours to fill in, and nothing on the form calculates. It is an ordinary Word file with no protection or form fields, so you click into a box and type.

The form opens on your letterhead, with NOTICE TO PROCEED, Notice no. and Date on the right. Four bands follow:

How to fill it in

  1. Replace YOUR LOGO and Your Company Name with your own, fill in the address and Phone · Email · License no., then type the Notice no. and Date on their lines.
  2. Under ISSUED TO, fill in the CONTRACTOR, the ADDRESS and the ATTENTION line for the person it goes to.
  3. Under PROJECT, fill in the PROJECT, the SITE ADDRESS, the AGREEMENT OR CONTRACT NO. and the AGREEMENT DATE. Copy the number and date exactly as they appear on the signed agreement. The form checks none of it.
  4. Under NOTICE, enter the START DATE, the COMPLETION DATE and the SITE CONTACT, and say when and where the PRECONSTRUCTION MEETING is.
  5. Mark RECEIVED BEFORE START for whatever has come in. The boxes are typed characters, not checkboxes you click: type an X over one, or tick it on paper.
  6. Sign under ISSUED BY, with SIGNATURE, PRINTED NAME AND TITLE and DATE. The recipient does the same under RECEIVED BY.

Using it on the job

The file has no example, so this section covers using it on the job.

Match the dates to your schedule. The START DATE and COMPLETION DATE should agree with the dates on your construction schedule. Give each notice its own Notice no. and keep a copy of every one you send, so the file shows what was sent when.

Fill in the boxes at the time. Tick RECEIVED BEFORE START as each item arrives, not all at once from memory. A blank box then tells you what is still outstanding.

Get the signed copy back. The RECEIVED BY block is the recipient's. Keep the returned copy with the agreement and the meeting minutes, so the three sit together in the job file.

Keep the form to what it prints. The work, the price and the schedule details belong on the documents built for them, such as your scope of work and proposal.

Frequently asked questions

Does it calculate anything?

No. It's a blank form. Nothing on it adds up or checks a figure.

Does it work in Google Docs or Pages?

Both open .docx files, and this one has no macros, form fields or content controls. Each app lays out Word files its own way, so check that the signature blocks still sit on the same page as the notice.

Can I put my logo on it?

Yes. Delete the words YOUR LOGO at the top left and insert your logo there.

Can I change what the form prints?

Yes. It's an ordinary Word file with tables and no protection, so you can edit any label or add a row. Check afterward that the form still fits one page.

Are the boxes clickable?

No. The three boxes are typed characters. Type an X over one, or tick it on paper.
